For clients 18+ A government issued photo I.D. is required for service. Listed below are the most commonly used: -Driver’s License -Passport -State I.D. For minors 10-17 years of age Parent or legal guardian must be present and have a valid form of identification. Minor must have a form of valid identification or a school I.D. Minor’s Birth Certificate or Proof of Legal Guardianship must be provided. Legal Guardianship needs to provide paperwork signed by a judge and be notarized. Last names on parent’s I.D. needs to be the exact same on birth certificate for minor. If they are not we need proof of name change. This means a photo of marriage certificate.
